Following the initiation of
bio-identical hormone replacement therapy, Moundsville Pharmacy will continue to
work with you and your physician regarding hormone replacement therapy
monitoring by evaluating your symptoms and addressing any questions or concerns you
may experience. Moundsville Pharmacy works one-on-one with patients to
provide the best care to encourage successful outcomes.
Moundsville Pharmacy has prepared follow-up evaluations to help evaluate your response
to
the therapy. Evaluations are scheduled at 30 days, 60 day, 90
days, 6 months, and 12 months following the initiation of therapy.
Patients are requested to fill
out and return these forms as scheduled so that
additional recommendations can be made to improve outcomes.

Following your first year of therapy, patients are encouraged to
retest their hormone levels through saliva hormone testing.
